What a Complete Referral Looks Like
To help us evaluate patients efficiently and determine the most appropriate treatment plan, we ask that referrals include the following information when available.
Recent imaging of the area of concern (within the past two years)
The last 2–3 office visit notes related to the area of concern
Physical therapy notes, including progress and outcomes
Prior pain management records, if applicable
Records from related specialty care, such as orthopedics or neurology
Providing this information at the time of referral allows our team to review the patient’s history thoroughly, reduce delays, and ensure a more timely and effective care plan.
