How to Become a Patient

Call your insurance provider to confirm that Haven Spine+Pain is in-network for your plan.
Step 3
Wait for our team to contact you. If you have not heard from us within two weeks of your provider sending the referral, please call our office at 616.317.7246 so we can review your referral status.
